Logging in
At the top of this page, you will find an orange box with the button ‘Log in with DigiD’. This gives you easy access to your patient portal. As a young person (from 12 years old), you can log in to your record with your DigiD, using an SMS code or the DigiD app. If you do not yet have a DigiD, you can apply for one at www.digid.nl.
Do you want to log in as a parent or legal representative?
ParentYou need authorization. You can request this from the medical assistant at the department where your child is being treated.
You will need:
your child’s ID
your own ID
your child’s consent if they are older than 12
Are you 16 or older? Then you can authorize someone yourself via DigiDFrom the age of 16, any previously granted authorization automatically expires. Through DigiD Authorization, you can give online permission to someone you trust, such as one of your parents. That person can then log in to your patient portal with their own DigiD and manage things on your behalf.
You can request the authorization at machtigen.digid.nl. You can also revoke the authorization there at any time.
If the child is legally incompetent, the authorization can continue until the child turns 18, through the medical assistant. This remains as it is now.
Legal RepresentativeYou need authorization. You can request this from the medical assistant at the department where the child is being treated.
You will need:
the child’s ID
your own ID
an official decision from the child’s curator or guardian
the child’s consent if they are older than 12
Ask the doctor's assistant at the Princess Máxima Center for access to the portal. Once we have set up your access, you can log in with your DigiD. Access must be given physically at the Center itself. This is for safety and security reasons, to ensure that the right people are given access to the right records.
If your child is a patient at the Princess Máxima Center, you first require authorization to gain access to the portal. More information on this is provided elsewhere in these FAQs. You can download the authorization form here.
Yes, to ensure that your data remain secure and that only you have access to your environment, you must always log in with DigiD.
There are several possible reasons for this:
The Princess Máxima Center does not yet have your Citizen’s Identification Number (Burgerservicenummer (BSN)) on file or that of your child. Consequently, the portal remains closed.
You have not yet been authorised. If you have not yet completed the authorisation form, you cannot log in yet. You can fill this form at the central desk.
DigiD can malfunction.
Have you saved the link to your favorites? If so, we recommend you remove the cookies in the internet options and/or delete them in your browser's history/cache.
If this does not solve the problem, we recommend you phone the UMC Utrecht patient help desk (088 756 88 90).
Access and authorization
- Up to age 16
For children aged 0–12, a parent or caregiver can request authorization. For children aged 12–16, the child must also sign the form to give their consent. You can read how this works below.
Authorizing via one of the doctor’s assistantsAuthorizations require a form. You can download the form here or pick one up at the doctor's assistant desk on the 2nd or 3rd floor. Bring the completed form along with a valid ID to the doctor’s assistants. They will process the authorization as soon as possible. Once it has been processed, you can log in to the portal using DigiD.
Authorizations can only be submitted in person at the center, not by email or post. This is to ensure everyone’s safety and to make sure that the right people get access to the right medical file. Each parent who would like access to the file must go through this procedure.
From age 16From the age of 16, any previous authorization automatically expires. From this point on, only the child has access to the patient portal. The child can grant access via DigiD if they wish.
Authorizing via DigiDAre you 16 or older? Then you can authorize someone to log in to your patient portal on your behalf. This can be done via DigiD Machtigen. (DigiD Authorizations).
What is DigiD Machtigen?DigiD Machtigen allows you to give online permission to someone you trust, like one of your parents. That person can then use their own DigiD to log in to your patient portal and help arrange things for you. For example:
send an e-consult,
request a repeat prescription.
You can request the authorization at machtigen.digid.nl. You can also revoke the authorization at any time.
Medical information is strictly personal by law. That is why access to the portal can only be physically arranged in the Center, not by email or post. This is for safety and security reasons, to ensure that the right people are given access to the right records. Every parent/caregiver/guardian who wants access to the records must adhere to this procedure.
To gain access to the UMC Utrecht patient portal, patients must have a citizen service number. This also applies to children. If your child does not (yet) have a citizen service number, we cannot give you or your child access to the patient portal. However, you can ask for the child's physical medical file.
No. A DigiD user name and password are strictly personal. It is therefore not permitted to log in with someone else's details.
According to the Dutch Medical Treatment Agreement Act (WGBO), children from the age of 12 have a say in decisions about their care and treatment. Via an authorization form they can state whether they only want to access their patient portal themselves, or whether they want to give their parent(s)/caregiver(s)/guardians access as well. You can have your authorization reinstated on your next visit to the Princess Máxima Center on the condition that the child co-signs the authorization.
As from the age of 16, the authorization that was previously granted automatically expires. From that age on, only the child has access to the patient portal.
If the child is unable to give informed consent, the authorization can remain in effect until the age of 18, at which time the authorization expires and only (you as) a guardian or mentor can gain (temporary) access to the child's portal.
The portal is closed upon death. However, you can request a print-out or a USB stick containing the records by completing the 'Request copy of medical file' form together with the primary medical practitioner.

Appointments
There could be several reasons for this:
If you haven't filled out the authorization form yet, you cannot log in. You can fill this out at the central outpatient desk.
The appointments have not yet been definitively scheduled by the scheduling center.
During scheduling, appointments are inevitably changed in an effort to ensure they all slot in as accurately and effectively as possible. Our scheduling office will telephone, email or send out a letter once an appointment is definite. This communication is always correct and therefore leading. Still have any questions or concerns? The appointment is final as soon as you receive your appointment letter.
About the information in the portal
You can ask any questions you might have to the nurse at time of admission. If you are at home, consider whether the question can wait until your next appointment or whether you want to contact the primary medical practitioner or nurse specialist directly.
If your child needs repeat prescriptions, you can request them 3-4 working days before the visit, so the medication will be ready for you at the pharmacy. There are two ways to request it:

Select e-consult and choose "pharmacy" as the recipient.
Create a message, and attach a photo of your prescription if you have one.
We will ensure the medication is ready for you to pick up.
Do you not yet have a repeat prescription at home?
Go to: "Dossier" > "Medicines" > "Request repeat prescription".
Choose the department where your child is being treated as the recipient.
Fill in when you would like to pick up the medication under "comments", and note any special details, such as if your child has a feeding tube.
Without access to the patient portalYou can request access to the patient portal from the medical assistant at the Máxima Center.

Images are currently not uploaded properly in the UMC Utrecht portal. We are working hard to update the app to improve this functionality. Once visual material can be accessed again, we will inform you via the portal.
If you want to speak to someone about medical records, contact the primary care provider or nurse specialist via the telephone number or email address that you always use.
Some parts of the records are not displayed in the portal. If you cannot find information that you would like to see, you can report this to the primary medical practitioner during your next visit.
The doctor's assistant at the Princess Máxima Center can change your details for you at any time. You can also make changes in the portal. The menu on the left contains the heading 'Patient data' -> Personal settings. There you can change or add most information. Insurance details cannot be changed via the portal. Instead, they must always be passed on to the doctor's assistant.
Privacy and security
The data in the patient portal comes from the electronic medical record at the Princess Máxima Center. This record is only accessible to healthcare providers involved in the treatment. When authorization is granted, an authorized person can also view the patient’s medical record.
If you see data that does not belong in the medical record, report this to the lead physician as soon as possible.
